Your family's guide to behavioral health care.
Navigating behavioral health care services can feel overwhelming — especially when language, cost, and unfamiliar systems stand in the way.
Our bilingual care navigators offer immediate, short-term support — working alongside families in their language, on their terms — to assess needs, coordinate care, and remove the barriers standing between a child and the help they deserve.
Services are provided in Spanish and English, with interpretation available for Mayan languages.
Eligibility
whO WE SERVE
- Children, teens, and young adults up to age 25
- Fairfax County residents
- Spanish-speaking families who would benefit from additional support accessing behavioral health resources
How to start with Conexiones
Start a referral or call with referral information. Families can request services directly; youth over age 14 can self-refer.
Services are provided in the home, at school, other community locations, NVFS offices, or virtually with flexibility for early evening hours.
Conexiones is funded by Healthy Minds Fairfax and operates in partnership with HopeLink, which provides care navigation services to all other Fairfax youth seeking behavioral health support.
